Our Office Policies
Financial Information
Payment for your balance or any balance you are a responsible party for is required at the time services are rendered. If glasses or contact lenses are ordered, a minimum payment of half the balance is required before the order will be placed. The remaining balance must be paid in full before material (glasses or contact lenses) will be dispensed to the patient or their guardian/representative. For your convenience, we accept cash, personal checks, Visa, MasterCard, Discover, America Express, and Care Credit Cards.
Insurance Information
Our office also provides medically necessary eye exams and testing for conditions such as diabetes, glaucoma, cataracts, eye injuries, and other eye and visual health problems. These exams and procedures are billed to the patient's major medical insurance (Regence, Cigna, Aetna, UnitedHealthCare, etc.), not routine eye insurance. If the patient has a VSP Essential Medical benefit, this benefit can be used as a secondary insurance after billing the primary major medical insurance plan. If you have any copays or a deductible that have not been met for the insurance year, you will be responsible for any amount your insurance transfers to you. Our office does our very best to verify our providers are in-network providers with your insurance plan. On rare occasions, our office may unknowingly be out of network. If this does occur the patient agrees to pay all non-covered fees and balances. Our office encourages our patients to know their insurance plan and to be aware of benefits, copays, deductible amounts, and provider network status.
Please note, we do not accept OHP, Medicaid, and Kaiser Medical.
Cancellation and No-Show Policy
We kindly request at least 24 hours’ notice if you need to cancel or reschedule your appointment. This allows us to offer the appointment time to another patient. In the event you cancel or reschedule with less than 24 hours’ notice, or if you are more than 15 minutes late to your appointment, you will be subject to a $50 late cancellation charge/
no show fee for each occurrence. If you are charged a $50 fee, that fee can later be used as credit towards your appointment copays, glasses, or other patient responsibility balances. Please note this credit is non-transferable.
